Complete the form, print it out and mail it along with a copy of the current rabies certificate and if applicable, proof of spay/neuter, that applies to each animal you are requesting a license for. 

Make checks payable to Stafford County for the amount of $10.00 for unaltered dogs (unfixed) or $5.00 for dogs that have been spayed or neutered.  Three year tags (must have at least 2 years remaining on current rabies vaccination) are available at $15.00 for unsexed dogs and $30.00 for unaltered dogs.  Kennel license are only available at the Treasurer's Office with a letter of approval from the Department of Code Administration.
